You can provide a prescription in one of two ways. Please select one option below for obtaining your prescription:
1) Through our online ordering process:
By ordering the “consultation” on the “check out” page a practitioner will provided to you for a nominal fee of $20.00. After selecting this option you will be directed to a questionnaire form after you place your order. You must submit that form to complete the process. You will also receive an order confirmation e-mail with a link to the “health questionnaire”. Be aware that the $20.00 fee for the prescription service is non-refundable, even if the practitioner declines your prescription. (Rx must be received to ship order)
2) By submitting a prescription from an LHP of your own choosing.
Your Dr’s office can use their standard Rx form OR the prescription form here. You may submit the completed form by e-mail to "prescriptions@cesultra.com", or fax it to 844-270-7990. (Rx must be received to ship order)
